    After submission, you will receive an automated email with your digital patient consent form and your patient questionnaire. Please fill out and sign these documents. You will be able to upload them together with your medical information later to your personal space. For data security, do not send any documents containing your personal data or medical information via email. 

    The link to your personal space will be sent to you within the next 24 hours. 

    For added security, please note that you will need to confirm your email address. 

    We will be automatically notified of your progress.
